I hereby waive, release, and hold harmless Omega Jiu Jitsu, its owners, instructors, employees, agents, and affiliates from any and all claims, demands, actions, causes of action, or liability of any kind or nature whatsoever, whether known or unknown, arising out of or in connection with my participation in any program, class, or activity conducted by Omega Jiu Jitsu.
I acknowledge and fully understand that martial arts and related recreational activities involve inherent risks, including but not limited to the risk of serious injury, disability, or death. I voluntarily assume all such risks associated with participation, whether foreseen or unforeseen.
